Much has been written about the need for health organizations and agencies, insurance companies, to transform their approach to patient care, from a focus on curing the sick to keeping patients healthy (or keeping their conditions from progressing to a more serious level). It’s a daunting challenge, given that it involves unprecedented visibility into patients’ involvement with the many parties involved with their care, including general physicians, specialists, clinicians, administrators, insurance companies, government entities sponsoring national health plans and even patients’ own habits in their home and workplace.

This presentation will focus on how citizens, public and private providers and payers (insurances, goverment, employers) can improve clinical care by promoting efficiencies in three areas:

• Population health intelligence ‐ Using data to ensure people are getting the right treatment at the right times and helping those who are managing care to measure and understand outcomes.
• Condition management: Focusing on disease management, remote patient monitoring and the more proactive activities of health education.
• Personal Health and: Patients taking a greater role in tracking and managing their health and health information.

The presentation will explain that organizations need to start with the vision of integrated care rather than focus on point solutions. It will go beyond the conceptual discussion and into the steps – whether these are preliminary, tactical or strategic in nature ‐‐ that actual organizations have taken to creating patient‐centric roadmaps toward health and wellness that pull in all parties involved in patient care.

Rather than focusing on projects with long implementation times and high costs, it will present actionable steps healthcare organizations can take to build easy‐to‐implement, affordable, high ROI systems that span the entire health landscape and focus on improving patient health.
